Pathological and Statistical Studies of Koilocytosis in the Cervical Intraepithelial Neoplasia

Abstract
The pathological and statistical analysis of the koilocytotic lesions in the uterine cervical intraepithelial neoplasia were performed. Two hundred ninety eight cases of uterine cervical intraepithelial neoplasia were used and those were diagnosed in the Pusan National University Hospital and other hospitals in Pusan during the period from 1984 to 1988. The specimens were classified according to the pathological view point and the incidence of koilocytosis and atypical mototic figures were investigated in each histological subtypes of CIN. The results ard summarized as follows; 1. The mean age of CIN was 42.1 years of age in moderate dysplasia, 42.5 years in severe dysplasia, and 44.2 years in CIS. The mean age tends to increase according to the increase of severity of the lesion. 2. The incidences of koilocytosis were 49.2%(31/63) in moderate dysplasia, 45.2%(28/84) in severe dysplasia, and 32.5%(49/151) in CIS, and those are reversely correlated with the degree of dysplasia. The mean age of the koilocytotic cell positive groups of CIN tends to be lower than that of koilocytotic cell negative groups. 3. The positive rates of koilocytosis in the histological subtypes of dysplasia was 65.2%(30/46) in keratinizing type, 43.2%(32/74) in nonkeratinizing type, and 25.9%(7/27) in metaplastic type. The statistically significant high rate of positivity was noted in keratinizing type. The positive rates of koilocytosis in the histological subtypes of CIS were 0%(0/6) in small cell type, 25.0%(5/20) in pleomorphic type, 25.7%(19/74) in parabasal type, and 49.1%(25/51) in keratinizing type. The statisically significant high rate of positivity was noted in the keratinizing type of CIS. 4. The incidence rate of atypical mitotic figures in CIN were 23.8%(15/63) in moderate dysplasia, 52.4%(44/84) in severe dysplasia, and 53.6%(81/151) in CIS. The incidence rate was reversely correlated with the severity of the lesion. The reverse correlation was noted on the incidence rate of koilocytosis and that of atypical mitotic figures.
